Evaluation Services

Comprehensive Evaluations | $1250

  • Observations in 2-3 different environments
  • Standardized and non-standardized assessments
  • Detailed chart and medical history review
  • In-depth interviews with client, parents/caregivers, and other key individuals
  • Comprehensive written report with individualized recommendations, resources, and referrals

Feeding Evaluation l $1350

  • In-depth occupational therapy evaluation geared toward children with feeding and oral motor challenges. 

Focused Screening Evaluations | $500

  • Milestone Checker Evaluation – Age-based developmental screening with standardized testing and individualized play recommendations
  • Picky Eater Screener – Video review, questionnaire analysis, and targeted strategies for mild to moderate feeding concerns
  • Teen/Adult Sensory Processing Screening – Targeted assessment of sensory needs in work, home, and community environments
